In fact, the microsoft website might have templates for this very thing. You can buy blank CDs in bulk at any Staples or Best Buy. Just make sure to purchase CD-Rs and not CD-RWs. The RWs do not work in all players.
If you have a quality printer, you can print everything at home. If not, go to a Kinko’s or STaples and they’ll help you with everything. They’re great and fairly inexpensive.
One of the greatest wedding favors I ever received was a burned CD. Across the front, they simply wrote "Love" in a fancy cursive font with a black sharpie (I believe). The cover of the CD was Gustav Klimt’s Love painting.

First of all Congrats! Second I think that a personalized CD is a great idea. You can find cheap blank cd’s from the link below, it all depends on how many guests you have. In regards to labelling them, personally I think the best way to go is to design your own. Maybe put a picture of youself and your fiancee’ on the front with your wedding date? You can buy cd labellling paper from most stores and print them off yourself, you may also decide to use a "cd stomper" which will make sure that the labels are secure and neat. Whatever you decide, good luck!

I burned CDs at home and used brown lunch bags as covers. I just cut the tops off and slid the CDs in, stamped a pretty design on them, and pasted the song list on back. They were beautiful!
